Use this form to report an issue with the User Fee software application.

  • User Fee office staff
  • Individuals authorized by the User Fee office

How we process your request

Typical resolution time:

1-2 business days

Process:

After you submit your request, MIS will:

  • Review the request
  • Contact you for further information, if necessary
  • Examine/diagnose the problem, and troubleshoot until resolution
  • Send a closeout notification upon resolution
