Use this form to report an issue with the phone line going to a fax machine.

How we process your request

Process:
After you submit your request, MIS will:
- Review the request.
- Secure/Install any cable/connections needed.
- Verify fax machine's phone line is working as expected.
- Send a closeout notification upon resolution.
Before you submit a ticket, you will need to:
- Identify the room, and area where the fax machine is. (ex. Room 1301, fax is in the back north corner)
- Please add any pertinent details about the fax machine connection (ex: damaged, still working, etc.)
