Overview

Use this form to report an issue with the phone line going to a fax machine.

  • Any City of Buffalo employee

How we process your request

Typical resolution time:

3-4 business days

Process:

After you submit your request, MIS will:

  • Review the request.
  • Secure/Install any cable/connections needed.
  • Verify fax machine's phone line is working as expected.
  • Send a closeout notification upon resolution.

Before you submit a ticket, you will need to:

  • Identify the room, and area where the fax machine is.  (ex. Room 1301, fax is in the back north corner)
  • Please add any pertinent details about the fax machine connection (ex: damaged, still working, etc.)
