Overview

Use this form to add or modify a configuration(s) or Request Type(s) in the 311 software system.

Who can submit this request?

  • Department Heads
  • Supervisors
  • 311 Call Center Manager

How we process your request

Typical resolution time:

14-60 business days

Process:

After you submit your request, MIS will:

  • Review the request
  • Contact you for further information, if necessary
  • Create or modify configuration(s) / Request Type(s)
  • Send a closeout notification upon resolution

Before you submit a ticket, you will need to:

  • Determine if you need a new Request Type or a new configuration in the 311 system.
  • Provide all pertinent information required to create or modify a Request type(s) / configuration(s).
