How we process your request
Typical resolution time:Simple forms: 2-3 days; more complex forms with 20+ fields: 5-7 days
Process:
After you submit your request, MIS will:
- Review the paper form that was uploaded to the portal within 24 hours
- Reach out to gain any further qualifying information about the form and data collection needs
- Send a form to the department for review before it goes lives on the website
Before you submit a ticket, you will need to:
- Know what fields the online form will contain.
- Know the content (fields names/descriptive text) of the form.
- Scan a paper form you want to digitize or have an electronic copy to attach to this request form
- If requesting to have a brand new digital form (not from an existing paper form or fillable PDF), prepare a Word document that lists all information to be collected on the form to attach to your service request.
