Use this form to request assistance with BAS software used in the City Clerk's office.

How we process your request

Typical resolution time:

1-2 business days

Process:

After you submit your request, MIS will:

  • Review the request
  • Contact you for further information, if necessary
  • Examine/diagnose the problem, and troubleshoot until resolution
  • Send a closeout notification upon resolution

Before you submit a ticket, you will need to:

  • Provide as much detail about the issue as possible
  • Include full name in Issue Description field (if requesting new User ID)
