Use this form to request any type of phone records.

  • Department head or their designee

How we process your request

Typical resolution time:

1-2 business days

Process:

After you submit your request, MIS will:

  • Review the request
  • Use Verasmart software to produce the report
  • Send report and closeout notification upon resolution

Before you submit a ticket, you will need to:

  • Know extension numbers, time frames, type of call (incoming/outgoing) that you want to report on
