Overview
This form is used to remove employee access to the City's network and software programs. Use this form to eliminate access for both employees no longer working for the City of Buffalo and employees with a leave of absence.

- Department head or special assistant
- Human Resources (Civil Service/Compensation & Benefits)
- HR Commissioner
- Labor Relations
- Corporation Counsel

Process:
After you submit your request, MIS will:
- Review the request
- Cancel or suspend account access, as appropriate
Before you submit a ticket, you will need to:
- Know what access is to be canceled
- Know when access is to be canceled
