Name Change (Network ID/Username) Request

Overview

Use this form to request a name change to your network ID/username. City employees can request a change to their network ID to reflect a legal change to their first name or last name or both.

NoteThis name change request form is only for a name change regarding the network ID/username. If you need to change your legal name with the City of Buffalo, please contact your timekeeper or the Payroll department.

Who can submit this request?

  • Any City of Buffalo employee who needs to change their network ID/username due to a change in their legal first name or last name or both.
Note: Request may not be submitted on anyone else's behalf. You may only request a name change for yourself.

How we process your request

1-2 business days

Process:

After you submit your request, MIS will:

  • Review the requests
  • Make the requested changes to the system

Before you submit a ticket, you will need to:

  • Ensure the correct spelling of your new first name and last name (even if one of the names did not change)
