Use this form to request access to cloud-based file sharing websites for legitimate governmental purposes.

Note: Cloud-based file sharing poses significant risks to entire City of Buffalo network system. Organized crime and unscrupulous individuals leverage site access to import professional grade malware such as ransomware and identity theft to connected systems. You will need to provide business and operational justification for the increased risk and threat to this network from exposure to these sites.

Process:

After you submit your request, MIS will:

• Route to your supervisor for approval
• Review the request
• Enable access, as appropriate
• Send a closeout notification upon resolution

Before you submit a ticket, you will need to:

• Write a business justification describing why you need access to these service to perform your job duties
• Agree that the access requested is critical to current and future business operations.
• Agree that you submit the request with the full knowledge of the significant increased threat of professional malware to the City of Buffalo network and all the information residing on it
• Ensure the use of this additional access is in compliance with City of Buffalo business policies and professional conduct
• Agree that you will advise M.I.S. should this access no longer be required
