Use this form to return a device (such as Laptops, Tablets, Cell Phones, Wifi-Hotspots) back to the City of Buffalo.
Note: All other unneeded equipment should be dropped off in room 1211 at the designated drop-off area.

Process:
After you submit your request, MIS will:
- Review the request
- Record the intake of the devices
- Send a closeout notification upon resolution
Before you submit a ticket, you will need to:
- Please add any pertinent details about the equipment being recycled (ex: damaged, still working, etc.)
- Posts-its are provided to add contact information on returned equipment.
