Overview

Use this form to reinstate device (cell-phone, tablet, laptop) owned by the City of Buffalo that has been frozen or otherwise disabled.

  • Any City of Buffalo employee

How we process your request

Process:

After you submit your request, MIS will:

  • Review the submission
  • Log the device as registered to you
  • Re-enable the device.

Before you submit a ticket, you will need to:

  • Provide your name, department, supervisors name and phone number or device id of the device to be registered
