Transfer Employee Request

IMPORTANT NOTE: Before submitting a request to transfer an employee, employees that have not yet read the City of Buffalo Network System (CBNS) Network Policy and User Agreement must read and e-sign an acknowledgement that they have read and understand the policy and agreement. 

Please right-click on this link, click "Copy Link Address" and paste the link in an email to any and all employees that have not previously signed, asking them to read the network policy and e-sign the acknowledgement, and letting them know who they should be listing as supervisor on the form. If email isn't available, direct them to the link over the phone. Once e-signed, the supervisor they designate will receive a notification to proceed with filling out this employee transfer form on their behalf.

Overview

Use this form to request an employee transfer from one location to another and ensure they have the right access to the software programs and computer hardware necessary to perform their job duties.

Who can submit this request?

  • Transferring Employee's new Department head
  • Transferring Employee's new Supervisors
  • Transferring Employee's new Departmental designee

How we process your request

Typical resolution time:

2-3 business days

Note: If there is no existing LAN drop at the workstation and MIS is unable to install a new LAN drop, the department is responsible for reaching out to Wiretek Communications to have a new LAN drop installed. This may result in a slight delay in setting up the transferring employee's workstation.

Process:

After you submit your request, MIS will:

  • Review the request
  • Determine if transferring employee has e-signed the Network Policy and User Agreement Acknowledgement
  • Create a network account and email account (if applicable) for the transferring employee (if e-signature has been verified)
  • Grant access to all requested applications
  • Ensure requested hardware is installed before the employee start date

Before you submit a ticket, you will need to:

  • Ask transferring employee to read the network policy and e-sign the acknowledgement here (if applicable)
  • Wait for a notification that the transferring employee has e-signed the acknowledgement (if applicable)
  • Verify the transferring employee's name, title, organization, department, supervisor, building, and location
  • Conduct an inventory of hardware already at the transferring employee's workstation and make note of any hardware they will still need
  • Determine if the transferring employee requires a City email account
  • Determine what software systems they need access to and what type of access or permissions they need on those systems
