IMPORTANT NOTE: Before submitting a request for a new account, ALL account users must read and sign the City of Buffalo Network System (CBNS) Network Policy and User Agreement as well as fill out and sign the New User Account form.

Overview

Use this form to request a temporary service or vendor account with access to the City's network and ensure the account gets the right access to the software programs and computer hardware necessary to perform their duties.

Who can submit this request?

  • Department heads
  • Supervisors
  • Departmental designee

How we process your request

Typical resolution time:

2-3 business days

Process:

After you submit your request, MIS will:

  • Review the request
  • Determine if vendor has signed the Network Policy and User Agreement Acknowledgement
  • Create a network account and email account (if applicable) for the vendor (if e-signature has been verified)
  • Grant access to all requested applications

Before you submit a ticket, you will need to:

  • Ask vendor to read the network policy and sign the agreements
  • Verify the vendor name, title, organization, department, supervisor.
  • Determine if the vendor requires a City email account
  • Determine what software systems they need access to and what type of access or permissions they need on those systems
