Overview

Use this form to request a new desk (VoIP) phone.

Note: If your existing desk phone is not working, please fill out a "Report a Cell Phone or Desk Phone Issue" form instead. See related services below.

  • Any City of Buffalo employee

How we process your request

Typical resolution time:

1-2 business days

Process:

After you submit your request, MIS will:

  • Automatically route the request for department head approval
  • Review the request
  • Verify that an existing phone can't be repaired
  • Install new phone and configure for user

Before you submit a ticket, you will need to:

  • Provide building and room number where phone should be installed
  • Describe the reason for a new desk phone is needed
