Overview
Use this form to request hardware peripherals that are lost, broken, or missing.
Note: If existing hardware is malfunctioning, please submit a "Report a Hardware Issue" form by clicking the link in Related Services below. If the hardware cannot be repaired, it will be replaced.

- Any City of Buffalo employee
How we process your request
Typical resolution time:
3-5 business days
Process:
After you submit your request, MIS will:
- Review the request
- Determine if equipment is in stock on premises
- Order new stock (if item is not in current inventory)
- Install the new hardware in designated location
- Send a closeout notification upon resolution
Before you submit a ticket, you will need to:
- Know the name and office location of person receiving hardware (if requesting on behalf of someone else)
- List any peripherals required
