Replace Lost/Missing/Broken Hardware

Overview

Use this form to request hardware peripherals that are lost, broken, or missing.

Note: If existing hardware is malfunctioning, please submit a "Report a Hardware Issue" form by clicking the link in Related Services below. If the hardware cannot be repaired, it will be replaced.

Who can submit this request?

  • Any City of Buffalo employee

How we process your request

Typical resolution time:

3-5 business days

Process:

After you submit your request, MIS will:

  • Review the request
  • Determine if equipment is in stock on premises
  • Order new stock (if item is not in current inventory)
  • Install the new hardware in designated location
  • Send a closeout notification upon resolution

Before you submit a ticket, you will need to:

  • Know the name and office location of person receiving hardware (if requesting on behalf of someone else)
  • List any peripherals required
